HOSPICE FAQ

There isn’t a specific time for when to begin hospice. But there are some signs you can watch for that may help you make that decision.

  • Unexplained weight loss
  • Increased incontinence episodes
  • Decreased ability to perform or participate in daily activities such as bathing, dressing, and toileting
  • Increased fall or unsteadiness
  • Spending more time in bed, withdrawn
  • Decreased cognition, memory impairment
  • Your physician has told you that you might have 6 months or less

We are happy you are looking to stay within Summit’s continuum of care. If you are currently receiving services from our Home Care division, we can seamlessly transfer your care on to our Hospice division once you have been approved for hospice services. This process ensures a smooth transition to hospice without a lapse in support from our team.

A physician must certify that you have a terminal illness with a life expectancy of six months or less if the illness runs is natural progression and you have decided to decline or discontinue any curative or life sustaining measures, and pursue comfort care measures

Speak with your healthcare provider about hospice, and if they feel you may be eligible for hospice. If you do not have a healthcare provider, you can reach out to us directly and we can assist you with this process.

Palliative care and Hospice care both focus on comfort. While there are multiple differences between Palliative Care and Hospice Care, the most notable difference is you may continue to pursue curative treatments and interventions wile on Palliative Care. With Hospice Care, you have decided to forgo curative measures and treatments, or you may have been told there are no longer any curative interventions available to you.

Medicare, Medicaid and most private insurance or managed care plans pay for hospice. Most often, Medicare pays for hospice and is a dedicated benefit that covers all aspects of hospice care including durable medical equipment, medications related to the terminal diagnosis, and visits from all core disciplines necessary to provide care and support to your loved one. These disciplines include skilled nursing, medical social services, spiritual support and bereavement. Always check your coverage to confirm available benefits.

Summit is a proud partner of the We Honor Veterans program and can help with additional veterans benefits for end-of-life care.
